Across New York, free medically tailored meals and connected social care exist for Medicaid members — but too many people never hear about them. We're here to change that, one conversation at a time.

Reach Community Care is an independent community outreach organization. We are not a government agency, and we are not one of New York State's official Social Care Networks (SCNs). What we do is simpler: we meet people where they are, listen to what they need, and walk them to a Care Navigator inside a contracted SCN who can actually deliver help — entirely at no cost.
We lead with food because food leads everywhere else. A medically tailored meal is rarely just a meal — it's a foot in the door for housing help, transportation, and the kind of whole-person care that makes chronic conditions easier to live with.
